Moving On Plan
This form will ask you to agree with your adviser or key worker what support you need, but also what you will do to make your move work.
Why Use It?
Leaving the E2E Programme and moving on to an apprenticeship, a job or back to college is a big step. The Moving On Plan is to help make sure learners are given all the support they need to make this move successful.
When Is It Useful?
Ideally the Moving On Plan should be completed by the Key Worker and the Personal Adviser with the learner prior to them leaving E2E. It might be completed for example during their last review or on their last day.
Key Features
- Form content follows the equivalent original paper Passport.
Online Help is based on guidance produced to accompany this form in the original Passport.
- The Moving On Plan can be partially completed and returned to later to review and update.
- The Agreement section should be completed before data is shared. If it is not, then data collected on this form will not be transferred to another adviser / organisation on Referral.
- On Save a preview of the current held in the Moving On Plan is presented for viewing and printing.
